If you need to apply for a traditional card or replace your existing Library card, please contact us.
- If you are younger than 16, you'll need a parent or legal guardian to help with this form.
For identification, we accept:
- Driver's license
- State ID
- Passport
- Consular identification card
For address verification, we accept:
- Driver's license
- State ID
- Utility bill
- Current residential lease
- Postmarked piece of mail addressed to you
- Postcard mailed to you from the Library (ask us for details)
